Supplemental Services Packages

Supplemental Services Packages are based on: (1) A specific number of visits to the day and/or residential programs by MTRC's case manager; (2) Attendance at the annual team meeting where goals and objectives for the year are developed, and; (3) A specific number of telephone calls to services providers.

Written reports and recommendations are sent on a regular basis to the person you designate as "Trustee" of discretionary funds or other designated advocate or relative. MTRC will work with the designated person to resolve problems addressed in the reports.

When developing Life Care Plans, determine which package best fits the needs of the MTRC client, include it in the Care Plan, and make financial plans according to the choice.

The Supplemental Service Packages can be customized to fit the individual's needs. The packages are based on a reduced hourly fee, using today's costs, to save you time and money. The annual cost is subject to increase as MTRC's costs increase.

PACKAGE 1
QUARTERLY INTERVENTION
(Based on 43.5 annual hours)

  • 3 visits per year with written reports

ANNUAL COST: $2292

PACKAGE 2
QUARTERLY INTERVENTION
(Based on 29.5 annual hours)

  • 3 visits per year with written reports
  • Attendance at one Team Meeting with report
  • MONTHLY Phone calls to Service Providers
  • Review reports from Service Providers
  • Communication with Trustee/Advocate

ANNUAL COST: $1592
(A savings of $295 on the hourly fee)

PACKAGE 3
SEMI-ANNUAL INTERVENTION
(Based on 22.3 annual hours)

  • 1 visit per year with written report
  • Attendance at one Team Meeting with report
  • MONTHLY Phone calls to Service Providers
  • Review reports from Service Providers
  • Communication with Trustee/Advocate

ANNUAL COST: $1183
(A savings of $225 on the hourly fee)

PACKAGE 4

The examples given are not intended to be limiting, but only to suggest areas in which MTRC can be helpful

Concentrated Information Service - Provide qualified personnel to gather and evaluate information. Examples are:

Direct Personal Service - Assist in arranging for qualified personnel to provide a direct service. Examples are:

Professional Consultant Service - Assist in arranging for qualified professional for a specific need. Examples are:

Perpetual Extraordinary Service - Assist in designing a program of additional direct services to a beneficiary which will be fully funded by an outside source of funds which is to be implemented upon the death or disablement of the sponsor. Examples are:

Definitions:

Proponent - Any person or agency who has a legitimate interest in the welfare of the individual being served.
Visitor - A person selected and trained by MTRC to visit a member on a regular basis. This person usually will live in the geographic area of the member.
